Near-miss reporting by voice, investigations, CAPA and digital permit-to-work
EHS lets anyone report hazards, near-misses and incidents by voice or photo in their language, then runs investigations, CAPA, inspections and risk assessments. Digital permit-to-work checks competence, isolation and conflicts before a named issuer signs.
What it does
EHS is planned for W5 to bring environment, health and safety work onto the same records as sites, visitors, assets and people. Observations and near-misses — unsafe acts, unsafe conditions and good practice — can be reported on the web, mobile, WhatsApp (voice or photo), a QR code at the location or a kiosk, with an anonymous option per site policy. AI transcribes and translates voice, extracts who, what, where and when, and proposes a classification and potential severity with citations. A person confirms.
Incidents to verified CAPA
Incident reporting covers injury and illness, property damage, environmental release, fire, security and vehicle events, with minimal initial fields. Configurable classes (first aid, medical treatment, restricted work, lost time, dangerous occurrence) and jurisdiction packs support statutory reporting (to verify). Investigations use templates for 5 Whys, fishbone and barrier analysis. CAPA carries owner, due date, hierarchy-of-controls tag, evidence and an effectiveness verification step.
Control of work
Permit-to-work covers hot work, work at height, confined space, electrical isolation and LOTO, excavation, lifting and chemical handling: request, risk assessment, checks, issue, handover, suspend and close, with a named issuer and area owner. SIMOPS conflict detection blocks overlapping incompatible permits in the same zone and time. Isolation records capture lock numbers and zero-energy verification. Stop-work authority lets anyone stop work from mobile or WhatsApp and suspends linked permits.
Contractors, inspections and registers
Contractor pre-qualification, worker competence and induction validity link to the visitor gate. Inspections run from templates offline with scoring and photo evidence; failed items raise CAPA. HIRA and JSA risk assessments, toolbox talks, statutory registers, environmental consent conditions, chemical registers with SDS, PPE management, management of change and behaviour-based safety observations complete the module.
Safety stays human
The AI proposes and prepares. It never classifies final severity, issues permits or closes incidents. Safety KPIs use configurable formulas and leading indicators such as near-miss rate and CAPA on time.
